This plain language health care tool helps individuals living with intellectual disability think about their preferences with an array of medical decisions. This resource is meant to be filled out when an individual is healthy and well. Ideally, it would be completed before they encounter health challenges, and reviewed regularly to ensure it is an accurate representation of their wishes.
